At West Michigan Hospice, we’re like family. We’ll come to you in your time of need, and we’ll lend a helping hand to guide you through some of the most difficult decisions you’ll make. With a focus on providing in-home, quality-of-life care to West Michigan residents with chronic conditions, like Dementia, COPD, and CHF, our hospice team offers comfort, support, and clinical expertise to ensure you or your loved one can live their life to the fullest.

Meet Our Founder

Our founder, Aaron Leestma, has an extensive background in all areas of healthcare — including, of course, hospice care! Before founding West Michigan Hospice, Aaron served as an administrator for a skilled home health care agency for 20 years, supervising care for over 1,100 in-home patients per year. He has also previously served as an executive director for a Program of All-Inclusive Care for the Elderly (PACE), which provided him with skills and knowledge that span across the entire continuum of healthcare.

Our Core Values

At West Michigan Hospice, it’s our goal to provide compassionate and comprehensive hospice services to each of our patients and their families. In order to achieve this, our hospice care team operates with a dedication to maintaining these four core values:

Personalization

Our hospice team is dedicated to knowing all of our patients by name and providing them with a unique and comforting hospice care experience. Upon admission to our program, each of them receives their own personalized care plan that governs the hospice services they receive and the frequency in which they receive them.

Compassion

Above anything else, our team is dedicated to providing our patients with compassionate hospice care, and we often establish impactful relationships with them and their families. Upon hire, each of our team members commits to operating with humility, ambition, intelligence, and optimism. 

Collaboration 

Our interdisciplinary team (IDT) is composed of hospice physicians, hospice nurses, personal care aides, medical social workers, grief and bereavement counselors, spiritual care providers, volunteers, and more. We meet regularly to ensure the care our patients receive is appropriate, timely, and beneficial. 

Timeliness

In a crisis, we know that you need an attentive response. That’s why our hospice care services are available 24/7, even during the middle of the night. We always have a hospice nurse on call to attend to your urgent needs. 

Our Comprehensive Hospice Services

 With a full list of comprehensive hospice services, our hospice care team is able to help you or your loved one live as comfortably and peacefully as possible. Our physicians review each patient’s chronic symptoms and then create a personalized care plan outlining what services they’ll benefit from — from hospice nursing to medical social work, volunteer services, and more.
